    JOB SUMMARY

    In accordance with agency Mission, Vision, Values, and Goals, the Treatment Team Leader provides clinical guidance, supervision, and training for the team assigned to them. Provides intensive case management in consultation with team and insures all written documentation is clinically appropriate, correct, and meets agency standards. Interacts daily with team affairs, attends team meetings, and consults with the treating physicians and other members of the treatment team to provide quality care to the clients within the department.

    PRIMARY FUNCTIONS

    Hires, trains and provides clinical supervision and training for CSS’s and QMHP’s assigned to their team. Participates in the staff’s performance appraisal. Reviews all clinical reports and progress reviews and progress notes generated by their assigned staff.

    Provides direct services to the department’s assigned population of clients. Direct services include intensive problem-solving, case-management services, crisis-intervention and therapy as assigned.

    Participates in and follows all agency and department policies/procedures regarding treatment and documentation practices, adherence to DMH and Medicaid standards, QA/QI procedures, and emergency procedures. Reviews the treatment and documentation practices of their assigned team.

    Participates in weekly staff meetings, makes assignments, and completes intakes as needed. Assures that admission/continued stay criteria are met and properly documented on the appropriate forms.

    Develops initial and annual assessments and individual treatment plans for entire CSS caseload within 30 days of admission into the program for initial assessment and within 30 days of the due date on annual renewals. Also writes yearly critical intervention plans on high risk clients.

    Facilitates program, departmental and agency communication/operations through attendance at psychiatrist meetings, all-staff meetings, inter-departmental consultation or committee/task team as assigned.

    Follows all agency policies and procedures including emergency and corporate compliance policies/procedures 100% of the time.

    SECONDARY FUNCTIONS

    Rotates on-call duties as scheduled and responds to all crisis situations during office hours.

    Provides consultation/education for the community.

    Completes assigned intakes and initial evaluations from waiting list.

    QUALIFICATIONS

    Education:

    Master's degree in psychology, social work, counseling or closely related human services field.

    Experience:

    Must have at least 2 years of experience working with the assigned population (prefer 3-4 years).

    License, Certification or Registration:

    Preferred, but not required

    Special Skills:

    Thorough knowledge of current DSM

    Proven written/verbal communication skills (including accurate report writing)

    Interviewing and Problem-solving skills

    REQUIREMENTS FOR POSITION

    Must become Privileged staff according to Medicaid guidelines to provide services to assigned population. Must acquire and maintain training related to job performance including, but not necessarily limited to: Agency Orientation, Departmental Orientation, Program Orientation, CPR training (every 2 years), First Aid training (every 2 years), Non-Violent Crisis Intervention (every 2 years), Infection Control, Medication training, Cultural Diversity (6hrs. every 2 years),and Documentation training. Must complete 16 hours per year with documented attendance of professional development/training or education related to case management, community support and to the special needs of serving adults with serious and persistent mental illness.

    Maintain good standing membership within agency Professional Staff as a Team Leader.

    Access to dependable automobile with adequate auto insurance to make home visits.

    Available to carry departmental pager on a rotating basis, respond to emergencies as needed which may require home visits, crisis placement, or hospitalization.

    DEGREE OF SUPERVISION

    Receives supervision from Director of Adult Community Support. Treatment plans and assessments will require the physician's review and signature.

    POSITIONS SUPERVISED

    Clinical supervision of the team CSS’s and QMHP’s.
