    Title:

    JOB SUMMARY

    In accordance with agency Mission, Vision, Values and Goals, the Youth Community Support Family Therapist provides therapy to youth and their families. This includes individual, family therapy and group therapy/education; in addition to community support services; arranges and coordinates assessments of the individual's need for treatment and rehabilitation; coordinates and monitors service and support activities. The Family Therapist functions as the primary contact for individuals assigned, as well as for other agencies, within the framework of a team model.

    ESSENTIAL FUNCTIONS

    Provides Individual and family therapy to assigned caseload of clients. Therapy services may include individual, family, and group services assigned. Provides community support services, community based case management services to assigned population as defined by department. Assists assigned clients in gaining access to resources, both internal and external, including, but not limited to, rehabilitative, medical, social, educational, and psychiatric (as clinically appropriate). Will meet all productivity requirements.

    Appropriately documents all clinical/program activities according to quality standards as outlined by agency policy and the Department of Mental Health regulations. All documentation must be in the electronic medical record within 48 hours of providing service.

    Conducts program defined clinical assessments and treatment plans with children and families, adolescents, and transition youth/adults who are affected by behavioral health, addictions, and developmental disabilities. Clinical assessments include, but are not limited to: brief evaluations, and diagnostic formulation, family assessment, annual treatment plans renewals. Assessments and treatment plan will be completed for assigned caseload within 30 days of admission into program and within 30 days of the due date on the annual review.

    Serves as a vital treatment team member, the primary service provider and contact person for assigned clients. Acts as a liaison to the community, service organizations and programs on behalf of assigned clients. Participates in treatment team to coordinate services and conducts treatment planning/reviews for assigned youth/adults, and families, in a clinically appropriate fashion meeting required standards for content and time lines.

    Provides family counseling to assigned clients and their significant others/families.

    Participates in and meets ethical/agency standards regarding all treatment and documentation practices, Quality Assurance/Quality Improvement (QA/QI) procedures, utilization management, and program/client outcomes.

    Follows all agency policies and procedures including emergency and corporate compliance policies/procedures 100% of the time.

    SECONDARY FUNCTIONS

    Provides consultation/education for community.

    Serves on task teams and committees as requested.

    All other duties as assigned.

    QUALIFICATIONS:

    Education:

    Master's degree in family therapy or closely relate field.

    Experience:

    A minimum of two year experience, or two semesters of supervised practicum experience, in mental health and family therapy from an accredited university. Practicum experience which includes family and/or child therapy. Experience working with families of addicted individuals.

    Additional requirements:

    One year supervised experience in family therapy or close supervision with immediate supervisor until agency privileging standards are met.

    Licensure:

    LMSW, LCSW, LPC of PLPC

    REQUIREMENTS

    Providers must have at least 1- 2 years of experience working with children, youth and their families. Must maintain current license and complete all required continuing education in order to maintain that license.

    DEGREE OF SUPERVISION:

    Licensed/Certified Professional:

    Weekly supervision with director supervisor or other licensed/certified professional for one (1) month following hire; and monthly supervision thereafter for the duration of the provider's probationary period.

    POSITIONS SUPERVISED:

    Position does not require supervision of other staff, however, the Licensed and/or Certified Professional may opt to provide Licensure/Certification Supervision for licensure/certification candidates through mutual agreement of both parties and separate from CMHS, Inc. oversight or regulation (see Licensure Supervision Plan).
