    JOB SUMMARY

    In accordance with agency Mission, Vision, Values and Goals, this position serves as nursing staff within the program which serves individuals (adults, adolescents and children) who need pharmacological treatment or evaluation. This position fulfills Medication Clinic and extended hours First Responder Clinic responsibilities. Responsibilities include managing scheduled and walk-in clients for Medication Refills as authorized by providers, updating client treatment plans, federally controlled substance data for Clozaril, prior authorizations and providing treatments as delegated by providers and nursing staff. This position will also assist with the intake and assessment process of new clients utilizing the extended hours clinic and will have rotating evening/weekend responsibilities. Nurses are expected practice client care within their scope of practice and request assistance from appropriate physician and/or APRN as needed.

    PRIMARY FUNCTIONS

    Provides non-controlled medication refill management of walk-in and scheduled clients who have been approved by provider staff. Provider delegation of refill management is for interim client visits for clients stabilized on medications for a period, determined by the physician, and who do not require dosage or medication changes.

    Obtains vital signs, weight, height, and BMI, PHQ9, Risk Assessments, Supportive Care Plan as well as AIMS assessments, Metabolic Syndrome screenings and other required interventions as defined by regulations to assist with client flow in the Medication Clinic and document in the EHR each visit.

    Provides treatment planning to Medication Clinic clients by updating treatment plans according to DMH and Medicaid standards to keep consistent with current treatment. Ensures that client signatures are obtained and treatment plans are reviewed with the clients and are actively involved in the treatment planning process.

    Manages medications as delegated by insurance requirements and provider requests, prescription assistance medication, and obtaining prior authorizations for the Medication Clinic. Document interventions in the EHR and follows up timely on all insurance requirements.

    Reviews and monitors lab testing and Clozaril results to include reviewing vital signs and appropriate labs with physician and/or APRN and takes appropriate action for abnormal results. Refers clients for follow-up with their PCP as needed.

    Triages and returns client phone calls made to Medication Clinic. Consults with physician regarding current or possible problems.

    Provides current and competent client education regarding client health issues, medications and other treatments and procedures prescribed by CMHS providers.

    Documents completely and appropriately in, EHR, and other health information platforms as is required. Monitors documentation for agency standards compliance, accuracy, and productivity.

    Follows all agency policies and procedures including emergency and corporate compliance policies/procedures 100% of the time.

    SECONDARY FUNCTIONS

    Perform consultation and education duties internally as requested in area of medication, medication regimes, and physical management.

    Refers clients with medication change/dosage change requests and all clients with health/behavior changes directly to providers for immediate review.

    Assists in monitoring QA and chart audits in a professional arena.

    QUALIFICATIONS

    Education:

    LPN required: graduation from an accredited school of nursing, accredited college/vocational school and current professional registration in the State of Missouri, with confirmation from the Missouri State Board of Nursing.

    Experience:

    Prior experience with adult and youth psychiatric nursing is preferred.

    Licenses, Certifications or Registrations:

    Must maintain and verify current Licensure within MO State Board Requirements.

    Special Skills:

    Medication skill competency and good base knowledge of medical/surgical issues.

    Organizational and time management skills.

    Demonstrates working knowledge of the medical field and how it relates to mental health problems.

    Good communication and computer skills.

    REQUIREMENTS FOR POSITION

    This position requires working with adults of all ages and some youth. Ongoing competency shall be demonstrated by review of assessment skills and quiz on an annual basis, consultation with peers, consumer satisfaction surveys and review of documentation.

    Ability to work a rotating weekly schedule.

    Ability to ambulate quickly; travel to and from various locations.

    Good organization skills in order to provide multiple services and carry out nursing duties.

    Possession of concise and easily understood verbal communication to work with clients and team members.

    Ability to remain calm and alert in both busy or crisis situations, possession of good decision- making skills, good judgment and ability to respond to physical crisis situations.

    Ability to train and teach non-medical personnel about medication and psychiatric issues.

    Flexibility to alter day hours as necessary.

    Current certification in Cardio-Pulmonary Resuscitation (CPR).

    Must gain a minimum of 3 hours training, yearly, of professional development in Cultural Competency.

    Must gain a minimum of 30 hours training every 2 years in the related area.

    Must maintain a successful criminal background check and drug screen.

    DEGREE OF SUPERVISION

    Ongoing daily supervision by Medication Clinic Administrator and Medication Clinic Providers as needed.
