    JOB SUMMARY

    In accordance with agency Mission, Vision, Values and Goals, manages the Disease Management services within CMHS by completing outreach and ensuring clients are assisted with engaging in services. The Disease Management Outreach Specialist works as a team member within the Crisis Department to ensure client needs are met.

    PRIMARY FUNCTIONS

    In coordination with the Disease Management Team Leader manages all Disease Management (3700 and ADA) cohort lists. Coordinates and provides outreach and engagement services and collaborates with appropriate intake department to schedule an initial assessment into program clinically appropriate for the client’s needs (ADA or CPR).

    Provides and/or coordinates that all client case management needs are being met while ensuring they become engaged in services at CMHS.

    Attends all trainings , conference calls and community wide meetings associated with the Disease Management program and communicates with the Crisis Services Manager about new information and any concerns.

    Completes all required documentation for the Disease Management program, which includes but is not limited to: review and updating cohort lists, uploading CIMOR reports and supervising and tracking outreach and engagement efforts.

    Serves as a Mobile Crisis Response Therapist, during business hours, by providing immediate crisis intervention services to clients in the Eastern Jackson County area. Receives calls from the crisis system or CMHS staff and completes assessments as needed.

    Assists with involuntary commitments and submits proper documentation to Probate Court. As well as, providing education and guidance to families and community members regarding local resources for follow up.

    Follows all agency policies and procedures including emergency and corporate compliance policies/procedures 100% of the time.

    SECONDARY FUNCTIONS

    Provides backup for reviewing Commcare reports, and coordinates follow up for face to face assessments, if needed.

    Attends related meetings, serves on committees for CMHS and other entities as needed or assigned to ensure smooth program operations. Provides feedback as to the efficacy of the system.

    Backup for monthly crisis service logging and initial intake assessments when needed.

    All other duties as assigned.

    QUALIFICATIONS

    Education- Bachelors in Psychology, Social Work, or related field required.

    Experience- Minimum of two years’ experience in mental health or related field, one year experience working with SPMI adult population.

    Minimum of one year working with Crisis or Inpatient populations preferred

    Licenses, Certifications, or Registrations- Crisis services training and certification is preferred.

    REQUIREMENTS FOR POSITION

    Minimum two years working with adult SPMI population. On call crisis shifts may be required.

    DEGREE OF SUPERVISION

    Is supervised by the Outreach and Access Coordinator and receives a minimum of one hour per month of supervisory counsel.
