    Job Description

    JOB SUMMARY

    Provides telephonic outreach to patients to optimize medication use and promote improved health outcomes. Completes comprehensive medication reviews and other clinical/medication quality improvement interventions. Communicates with patients, providers, and health plans regarding medication therapies. The pharmacist must be able to work in a highly charged, fast paced call center environment as well as participate in the further development and refinement of work systems, tools, processes, protocols, and procedures.  The pharmacist will be paid per successful case plus an hourly rate.  

    **THIS POSITION WILL BE LOCATED IN OUR ORLANDO OFFICE AT 6021 RIO GRANDE AVENUE, ORLANDO FL

    THIS POSITION INCLUDES, BUT IS NOT LIMITED TO, THE FOLLOWING ACTIVITIES AND RESPONSIBILITIES:

    • Performs patient medication therapy monitoring and disease state management counseling
    • Performs comprehensive medication reviews and other health care quality improvement interventions
    • Initiates prescriber communication regarding suggested changes for medication therapies
    • Partners with the patient to solve for non-adherence issues as well as work with the patient and the prescriber to assess the need for supplemental medications based on the unique needs of the patient and their disease state
    • Proactively conducts analysis of care setting drug utilization data to identify trends, develop strategies, and implement comprehensive programs to improve patient outcomes
    • Advises patients about general health topics such as diet, exercise and managing stress or refers to appropriate professional
    • Promotes medication safety and compliance
    • Supports and contributes to EMS internal quality performance and improvement initiatives
    • Other duties as assigned

     

     

    Requirements

     

    REQUIRED QUALIFICATIONS:

    • Current license to practice pharmacy in the state of Florida
    • Strong understanding of the major pharmaceutical computer software platforms
    • Excellent verbal and written communication skills
    • Strong customer service abilities

    PREFERRED QUALIFICATIONS:

    • PharmD
    • Experience working in a telehealth environment
    • Medication Therapy Management (MTM) experience preferred
    • MTM Certification
    • Ambulatory care/Geriatrics Residency
    • Board Certification in pharmacotherapy, ambulatory care, geriatrics
    • 2 or more years of clinical and/or managed care experience, Medicare Part D experience preferred

    WORKING CONDITIONS:

    • Incumbent must be able to work nights, weekends, and holidays as required by client needs
    • Incumbent must be able to work with challenging clients to resolve customer service issues and complaints
    • Incumbent must be able to work in a call center environment

    PHYSICAL REQUIREMENTS:

    • Incumbent must be able to remain seated for long periods of time, talk on the phone and perform computer data entry for long periods of time
    • Incumbent must be able to speak, hear, see, read, write, type, and dial
