    SUMMARY:    The health coach educator utilizes a collaborative process of education, assessment, planning, implementation and evaluation, to engage, educate, and promote/ influence program participants in decisions related to achieving optimal health status. Health coach educators are responsible for conducting onsite educational sessions following established program protocol at assigned program locations, as well as providing personal health coaching according to population desires and needs.

     

    RESPONSIBILITIES:

     

    1. Responsible for delivering established program curriculum in person at assigned onsite location(s) and/or during telephonic session engagement depending on participant preference.

      • Work conducted out of employer onsite locations as much as 75% of time.
    2. By assimilating health assessment data analysis, establishes a personalized plan of care based on participant’s health status/risks, education, and support needs.

      • Using holistic approach to wellness, derives strategies to overcome participant barriers in meeting goals and objectives to reduce type two diabetes risks.
    3. Utilizes motivational interviewing skills to ensure maximum participant engagement and provides coaching, information and support to empower participants to make ongoing independent healthy lifestyle choices.

    4. Through the use of clinical tools and information/data review, uses knowledge and experience of lifestyle risk behaviors to conduct comprehensive evaluation of participant’s health status, health literacy, lifestyle risks and readiness to change as they relate to type 2 diabetes.

    5. Displays ability to follow established program workflow and accurately document participant data into electronic chart in a timely fashion.

    6. Performs other duties as required.

     

     

    Required Skills

    QUALIFICATIONS:

     

    1. Bachelor’s Degree in health related field (e.g., Health Education, Wellness, Health Promotion) - Master’s Degree or Certified Diabetes Education credentialing preferred.
    2. 3-5 years of coaching experience – Health Coach Certification desired.
    3. Bilingual Spanish preferred.
    4. Background in motivational interviewing, active listening skills, Transtheoretical Model, mindfulness based and other evidence-based coaching techniques preferred.
    5. Experience in wellness related areas such as weight management, nutrition, physical activity or health lifestyle behavior change.
    6. Superior presentation skills required, including ability to deliver content in an engaging way and adapt to audience needs and/or questions.
    7. Experience in facilitating patient/client positive behavioral change.
    8. Experience in facilitating educational sessions, both in person and via technology modalities.
    9. Flexible schedule required.
    10. Ability to work with a multidisciplinary team.
    11. Excellent computer skills including MS Office applications, online charting systems and electronic medical records systems.
    12. Demonstrated ability to communicate, problem solve, and work effectively with people, use good judgement in challenging situations.
