Constant Care Health is seeking a fulltime:

Licensed professional clinical counselor (LPCC),

Licensed clinical social worker (LCSW),

Licensed mental health counselor (LMHC),

Masters level licensed alcohol and drug abuse counselor (LADAC),

Masters social worker (LMSW).

• Therapists will provide individual telehealth therapy and maintain accurate clinical records including progress notes and treatment plans.

. As a telehealth nurse you will be responsible for a group of patients that you will provide support to on a monthly basis. You will be responsible for building and maintaining a rapport with your patients while coaching them to improved health.

· Provide education and coach patients on strategies for self-management to keep patients out of the hospital.

· Connect the patience with community resources when needed.

· Utilize our chronic care management software to call patients and to aide and support them through their current patient care plans.

· Implement and improve the current plan of care by updating medications, symptoms, appointments, biometrics, and additional interventions made.

· Close care gaps by assisting and communicating preventative care techniques.

Job Type: Full-time

Salary: $35.00 - $40.00 per hour
