We are seeking a Contract Community Outreach Coordinator to help us build relationships with case managers, referral sources, and community organizations. The ideal candidate will be personable, self-motivated, and experienced in community engagement, particularly within the healthcare or social services field.
This is a 1099 contractor position, which means you are responsible for your own taxes and work schedule. The initial contract term is 60 days, with the opportunity for renewal based on performance and client growth.
Key Responsibilities
Identify and reach out to case managers, discharge planners, and referral coordinators in hospitals, clinics, and community agencies
Attend local health fairs, events, and provider meetings to represent the company
Distribute marketing materials and maintain contact with referral sources
Report weekly outreach activity and potential referral leads
Help generate interest and inquiries from potential clients or families
Collaborate with company leadership on outreach strategy and improvements
Qualifications
Experience in healthcare outreach, social work, marketing, or case management preferred
Strong communication and relationship-building skills
Self-starter with excellent organizational habits
Knowledge in the home healthcare field and referral systems is a plus
Reliable transportation for community visits (mileage reimbursement available)
Contract Details
Independent Contractor (1099) status
Initial 60-Day Trial Period – contract may be extended or terminated based on performance
Compensation: $20–25/hour (depending on experience and qualifications)
Hours: Flexible, up to 20 hours/week
Contract can be terminated by either party with 7 days’ notice
