Position Summary
The Licensed Vocational Nurse (LVN) is a contract position that is responsible for providing nursing care and health oversight for individuals with intellectual and developmental disabilities (IDD) in group home and day program settings. The LVN works under the direction of the Registered Nurse (RN) and in collaboration with the interdisciplinary team to ensure high-quality, person-centered care and compliance with HHSC and HCS program regulations.
Key Responsibilities
Nursing Care & Health Oversight
- Administer prescribed medications and treatments in accordance with DON and physician orders.
- Monitor and document clients’ health status, reporting changes promptly to the RN, physician, and case manager.
- Perform routine health assessments, including vitals, skin checks, and chronic condition monitoring.
- Ensure timely completion of nursing documentation (MARs, progress notes, incident reports, etc.).
Compliance & Safety
- Maintain compliance with HCS, HHSC, and state nursing regulations.
- Assist with infection control measures, health and safety protocols, and emergency preparedness.
- Participate in audits, surveys, and quality assurance reviews, ensuring readiness and accurate documentation.
Training & Support
- Provide health-related training and in-services to Direct Support Professionals (DSPs) and other staff.
- Educate clients and families on medication adherence, nutrition, hygiene, and health maintenance.
- Support the RN with care planning, Individual Program Plans (IPPs), and client-specific health protocols.
Team Collaboration
- Work closely with the RN, case managers, program managers, and direct care staff to coordinate holistic care.
- Communicate effectively with guardians, physicians, therapists, and other stakeholders.
- Advocate for clients’ health needs while promoting independence and dignity.
Qualifications
- Current Texas LVN license in good standing.
- Experience in IDD, group home, or long-term care settings preferred.
- Strong documentation, organization, and communication skills.
- Ability to work independently and as part of a team in a fast-paced, compliance-driven environment.
- CPR and First Aid certification required.
Work Schedule & Reporting
- Reports directly to the Registered Nurse (RN) and Program Administrator
- Flexible schedule may include weekends, evenings, and on-call responsibilities as assigned.
