TITLE OF POSITION: OUTREACH CASE MANAGER
Duties and Responsibilities:
The Outreach Case Manager will:
Provide case management services to clients living with/at high risk HH who have multiple psychosocial and/or health related needs, and promote health, wellness and recovery, by collaborating with community partners staff to outreach to potential clients, screen for eligibility, provide IDDT based case management, linkage to community-based
Participate in all program training;
Implement principles of person-centered, “stages of change wise” treatment planning, and trauma informed care;
Work as a part of a Case Manager/Peer Recovery Coach team and share a caseload of 30-35 clients;
Document services provided, keep treatment records and prepare service delivery reports.
Participate as a member of the wrap-around team
Provide community resource linkages to clients including: mainstream benefits, housing, education, employment, and other support services as identified
Provide treatment adherence counseling, coordination and follow-up of medical treatments, client advocacy, and assistance in obtaining housing, financial support, legal services, employment, social support and any other needed service

Supervisory Relationships:
Outreach Case Manager works under the direction of the Project Director. They are required to function independently, but must provide regular updates and status reports regarding contact with clients in the program.
Skills, Knowledge and Prior Experience Required: Experience working with individuals with chronic health conditions, HH programs, SMI/SUD/DD; community outreach; demonstrated skills working within the Latina/o and other minority communities.
Amount of Travel and any other special conditions or requirements: Working conditions include travel to and from the program and scheduled activities, appointments, etc. This person will also be required to make occasional home visits/hospital visits. This job entails high risk exposure to at risk population in high crime neighborhoods. This position involves being exposed to smoking environments.

Required Skills Required Experience
Bachelor’s degree from an accredited college/university and 9 years of related experience is required. Ability to obtain and maintain an active security clearance.
