Responsibilities: • Develops and assures that rehabilitation services meet the needs of the individuals served by the program. • Implement the program’s rehabilitation services, and educate direct care staff on how to provide rehabilitation services that correlate with the goals identified in the individual's Individual Rehabilitation Plan (IRP) or Individual Treatment Review Plan (ITRP). • Responsible for providing assessments and Individual Rehabilitation Plans (initial and concurrent) for identified consumers. • Will oversee the discharge process and re-engagement letters. • Provide and complete clinical documentation in a timely and accurate manner using electronic health records. • Maintain accurate and up-to-date records. • Participate as a member of the PRP staff with consultations, meetings, and supervision as scheduled. • Review monthly contact notes. • Screen consumers for the program • Other duties may be assigned by the supervisor Qualifications: • Must be excellent at communication and interpersonal relationships • Must have licensure that complies with state and local laws • To perform this job successfully an individual must be able to perform essential responsibilities satisfactorily. • Must have an active LGPC, LCPC, LMSW, LCSW-C • Effective written and oral communication skills • Preferred to have CPR certification • Proficiency with electronic health records Compensation: $30.00 - $34.00 hourly
