Remote Full Time CMA-AAMA or LPN for Chronic Care Management Program
Cullman Internal Medicine is seeking a full time CMA (AAMA) or LPN for our Chronic Care Management program.
Candidates should have the following:
Current Alabama nursing License or CMA-AAMA certification
Minimum two years clinical experience
Up to date knowledge of ICD-10 codes
Knowledge of insurance/clinical requirements for prior authorizations
Excellent written and verbal communication skills
Previous experience with a Chronic Care Management program a plus
CHRONIC CARE MANAGER / CERTIFIED MEDICAL ASSISTANT The Chronic Care Manager is a medical assistant who supports the development of patient-centered, team-based care. S/he will support primary care physicians (PCPs), specialists, and healthcare organizations in managing their panel of patients using a population management informatics & communication tool. By proactively engaging with patients over time, the Chronic Care Manager works to identify patients’ unmet needs, engage patients in their own care, gather summary information for treatment interventions, and enhance ongoing communication between the patient and their care team. The goal of the Chronic Care Management program is to facilitate high-value, patient-centered care that improves timely access to and provision of preventive services and chronic disease treatment.
YOU WILL BE RESPONSIBLE FOR: ● Developing a proper understanding of chronic pain and medical care requirements for optimal, coordinated population health ● Proactive patient management via phone calls ● Triage and assessment of patient health status ● Coordination of care between patient and provider ● Active review of patient charts to assess health needs and gaps ● Providing a positive patient experience for patients and families through courteous digital interactions ● Routing patient escalations accurately and expeditiously ● Making referrals to appropriate staff when necessary
LICENSE ● Certified Medical Assistant (Preferred) ● LPN
QUALIFICATIONS ● 2+ years of MA experience ● 2+ years of experience in a primary care setting or similar specialty ● 2+ years of triage experience ● Prior use of electronic health records (preferably Systemedx) and other health care information systems
