    We are looking for a Part-time Physical Therapist to join our medically integrated healthcare team. As a medically integrated praxis, we provide Physical Therapy services as an option to our patients who needs a more specific, defined, and narrower focus of care. We are committed to helping our patients prevent further injury and rehabilitating their injury/condition. $60 - $65 hourly Depending on experience -Sign on bonus! Responsibilities: • Completes assessment and evaluation of patient's impairments and deficits. • Working in partnership with our current therapist. • Creates a treatment plan and goals to address the impairments and deficits with consideration of evaluation and assessment, medical status, and prior level of function. The treatment plan and goals are monitored and adjusted based on the patient's needs and progress. • Advises patient on proper form and makes sure the patient will not injure themselves during exercise. • Instruct, encourage and assist patients in performing a therapeutic activity, therapeutic exercise, and activities of daily living, specific to each patient's needs and goals. Adjusts selected therapeutic activity, therapeutic exercise, and activities of daily living based on the patient's response to the activity. • Evaluate the effects of treatments by observing, noting, and evaluating the patient's progress and recommending adjustments and modifications in consultation with other providers. • Prepares written documentation such as notes, reassessments, and discharge summaries. Documentation includes monitoring the patient’s status and progress toward goal achievements. Re-evaluations are completed in a timely manner. • Goals and treatment plans are updated as per department policy. Consistently, accurately, and independently prioritizes and completes routine assignments within guidelines and schedules. Qualifications: • Must be board-certified and licensed in Physical Therapy by the New Jersey Board of Medical Examiners. • Possess excellent interpersonal and communication skills, with the ability to empathize with and relate to patients. • Displays energy, positivity, and excellent team-working skills. • Highly organized and capable of juggling multiple responsibilities. Compensation: $60 - $65 hourly DOE
