    Job Overview:

    Dependable Home Health of Phoenix searching for an experienced and dedicated PRN Medical Social Worker to join our team. The ideal candidate will provide medical social work services to our patients and families in a home care setting. The responsibilities of the Medical Social Worker include assessing the patient's psychosocial needs, developing a plan of care, and coordinating community resources.

    Key Responsibilities:

    • Conduct Psychosocial assessments and evaluate the patient’s social, emotional, and financial needs

    • Collaborate with the healthcare team to develop an individualized care plan

    • Advocate for patients’ interests and help them understand and access available community resources

    • Provide counseling services and emotional support to patients and families, to assist them in coping with illness and life changes

    • Communicate with healthcare providers, family members, and community organizations to coordinate discharge planning and after-care services

    • Maintain appropriate documentation and records for each patient within Home Health software system

    • Participate in quality improvement initiatives to improve patient outcomes and enhance program effectiveness

    • Attend and participate in team meetings, case conference, and in-services as required

    Qualifications:

    • Master’s Degree in Social work or related field from an accredited institution

    • A minimum of 2 years’ experience in Home Health or related setting; preferred

    • Current License to practice Social Work in the state of practice

    • Excellent communication, problem-solving, and organizational skills

    • Knowledge of Federal, State, and Local Social Service programs, regulations, and policies

    • Compassionate, caring, and respectful attitude toward patients and their families

    • Ability to work independently and a willingness to travel autonomously

    • Must have a current driver's license, reliable transportation, and proof of auto insurance
