Dependable Home Health is seeking highly skilled and compassionate Full-Time Home Health Float Pool RN's to join our dynamic healthcare team. As a Float Pool RN, you will provide exceptional in-home healthcare services to a diverse patient population across various geographical locations, emphasizing the importance of patient safety, comfort, and wellbeing. Responsibilities:
1. Conduct comprehensive patient assessments and develop individualized care plans based on patient needs, medical history, and physician instructions.
2. Administer medications, treatments, and therapies as prescribed by the physician, ensuring compliance with all applicable regulations and guidelines.
3. Collaborate with interdisciplinary healthcare team members, including physicians, therapists, social workers, and caregivers, to provide comprehensive patient care and ensure continuity of care.
4. Educate patients and their families about various health conditions, medication management, self-care techniques, and community resources to assist in their long-term wellbeing.
5. Perform skilled nursing procedures, such as wound care, catheterization, tracheostomy care, and IV therapy, in adherence to established protocols.
6. Conduct periodic visits to assess patient progress, update care plans, and coordinate care transitions as needed.
7. Document all patient encounters, interventions, and outcomes accurately and timely in electronic medical records (EMR), ensuring compliance with HIPAA regulations.
8. Collaborate with internal teams to effectively manage emergency situations, resolve conflicts or issues, and improve patient care outcomes.
Requirements:
1. Valid and unrestricted Registered Nurse (RN) license in the state of Arizona.
2. Proficient in conducting comprehensive patient assessments, developing care plans, and providing skilled nursing procedures.
3. Strong communication and interpersonal skills to effectively interact with patients, families, and interdisciplinary team members.
4. Ability to work independently and efficiently manage time, prioritize tasks, and meet deadlines.
5. Knowledge of applicable state and federal regulations, including HIPAA and OSHA guidelines.
6. Strong critical thinking, problem-solving, and decision-making skills.
7. Willingness to travel extensively, often to remote or rural areas, and work flexible hours, including weekends or evenings, based on patient needs.
8. CPR certification; additional certifications such as wound care, IV therapy, or advanced cardiac life support (ACLS) preferred.
