Home Health RN - Tucson South Side - Retention Bonus!
Dependable Health Services is looking for a dedicated and skilled Home Health Registered Nurse (RN) to join our team. In this role, you will provide compassionate care to patients in their own homes. As a Home Health RN, you will be responsible for developing and implementing individualized care plans to meet the physical, emotional, and spiritual needs of patients. You will work alongside other healthcare professionals including physicians, therapists, and social workers to ensure the best possible outcome for patients.
** Responsibilities:**
- Conduct home health assessments and develop care plans based on patient needs.
- Administer medications, treatments, and therapies as ordered by the physician.
- Provide wound care and disease management to patients with chronic illnesses.
- Monitor patients' progress and adjust care plans as necessary.
- Provide patient and family education to promote wellness and independence.
- Supervise and document the work of licensed practical nurses (LPNs) and home health aides (HHAs).
- Participate in interdisciplinary team meetings to discuss patient care and progress.
- Maintain accurate and timely documentation of patient care.
_ Requirements: _
- Current Registered Nurse (RN) license in good standing in the state of Arizona.
- A minimum of one-year experience as an RN, preferably in home health, hospice, or critical care settings.
- Current CPR certification.
- Strong communication, interpersonal, and organizational skills.
- Ability to travel to patients' homes and work independently.
- Must possess a valid driver's license and reliable transportation.
