    Job Description

    Dignity Adult Day Health Care Center  is a Community Based Adult Services licensed program that provides services to eligible older adults and/or adults with disabilities to restore or maintain their optimal capacity for self-care and delay or prevent inappropriate or personally undesirable institutionalization.

    1. As a member of the multidisciplinary team for dual ADHC, will conduct initial assessments and reassessments of all participants.

    2. Will develop and implement individualized plan of care based on the participant's assessment, ONLY APPLIES TO ADHC COMPONENT OF THE PROGRAM. The activity coordinator will involve the participants in the program planning process.

    3)Will  complete quarterly progress notes on each participant, only applies to ADHC component of the program.

    1. Plan and implement all activity programs at the Center.

    2. Develop and post a monthly calendar of events.

    3. Schedule other staff and volunteers for leading and co-leading activity programs.

    7)  Order supplies, keeps records of activity expenses, makes recommendations for supplies, materials, and equipment that will enhance the programs benefit to the participants.

    1. Maintain appropriate record keeping and documentation of participants response to activities offered and attended.

    2. Participate in staff in-service and other educational and training opportunities. 

    3. Conform to all applicable policies of the Center and attend the orientation program offered by the Center.

    4. Comply with all health and safety standards including licensing, fire and disaster preparedness regulations and agency policies.

    5. Attend all staff and other scheduled meetings.

    6. Take responsibility for developing activity programming that contributes to and enhances the goals of restoration to self-care and to prevent further deterioration.

    Qualifications:

    1. Have two years of experience in a social or recreational program within the past five years, one year of which was full-time employment in a patient activities program in a health care setting.

    2. Be an Occupational Therapist, music therapist, dance therapist, art therapist, recreational therapist, or occupational therapy assistant, or, have completed at least 36 hours or training in a course designed specifically for the Activity Coordinator position and receive regular consultation from an Occupational Therapist, occupational therapy assistant or recreation therapist.

    3. Flexibility, initiative, creativity and excellent organizational and time management skills.

    4. Prefer bilingual.

    5. Proficient in utilizing computer programs.

    Activity Assistant is a full time or part time position based on center needs and reports to the Program Director.
