Key Responsibilities:
Individual Support
Assist with daily living activities (hygiene, feeding, medication, mobility)
Support individuals in achieving personal goals and community engagement
Promote rights, dignity, and choice in accordance with HCBS standards
Ensure safety and well-being while providing individualized care
Program & Documentation
Accurately document services, incidents, and daily activities
Administer and document medications as required
Report incidents, concerns, or rights violations promptly
Maintain clear communication with team members and families
Professional Conduct
Maintain respectful, professional relationships with individuals and stakeholders
Demonstrate initiative, flexibility, and teamwork
Model positive behavior and uphold confidentiality standards
Operations & Safety
Follow all CAN policies, procedures, and regulatory requirements
Maintain health, safety, and cleanliness standards
Safely operate program equipment and vehicles
Training & Compliance
Complete required trainings and maintain certifications
Participate in ongoing staff development and HCBS training
Qualifications:
High school diploma or equivalent (required)
Minimum 1 year of related experience preferred
Knowledge of IDD and person-centered practices
Valid driver’s license with safe driving record
Physical Requirements:
Ability to lift at least 25 lbs
Ability to stand, walk, bend, and assist individuals as needed
Work Environment:
Must follow Universal Precautions and safety protocols at all times
