We are seeking a Designated Coordinator to join our team. In this role, you will be responsible for coordinating, supervising, and evaluating the delivery of person-centered services in accordance with Minnesota Statute 245D.081 Subd. 2. This includes ensuring that individuals receive services as outlined in their support plans, that staff are trained and competent, and that service delivery achieves measurable and observable outcomes.
Key Responsibilities
Coordination of Individual Services
Oversee implementation of each individual’s support plan and support plan addendum.
Facilitate service delivery to ensure progress toward measurable outcomes.
Provide instruction, assistance, and support to Direct Support Professionals (DSPs).
Conduct direct observation of services or delegate to a competent staff member.
Evaluate service delivery methods and adjust supports as needed to improve outcomes.
Staff Competency and Support
Assess and support DSPs in understanding and implementing support plans.
Monitor and verify competency through observation and documentation.
Train or delegate training to competent individuals in delivering specific services.
Work collaboratively with the Designated Manager and other supervisory staff to ensure quality service delivery.
Documentation and Evaluation
Maintain thorough records of staff competency, individual outcomes, and service delivery.
Analyze progress using measurable criteria as required by 245D.07.
Participate in internal reviews, audits, and quality improvement processes.
Develop and evaluate support plans and addendums.
Develop and evaluate risk management and behavior support strategies.
Develop and evaluate service outcome tracking systems.
Candidate Requirements
Must meet ONE of the following:
Bachelor’s degree in a human services, health, or education field and 1 year of full-time direct care experience with persons with disabilities or older adults;
Associate’s degree in a related field and 2 years of full-time direct care experience;
Diploma from an accredited postsecondary institution in a related field and 3 years of full-time direct care experience.
Minimum of 50 hours of education/training related to human services/disabilities and 4 years of full-time direct care experience under the supervision of a qualified staff.
Required:
Must be knowledgeable in Vulnerable Adult Act
Strong understanding of person-centered practices.
Ability to develop, implement, and evaluate support and outcome plans.
Knowledge of state regulations under 245D and DHS licensing.
Strong communication and documentation skills.
Proficiency with data tracking systems and electronic documentation.
Working Conditions
Monday-Friday in-office, with frequent visits to residential and community settings.
On-call or flexible hours as needed for service oversight
