    Job Summary

    We are seeking a Designated Coordinator to join our team! As a Designated Coordinator, you will work closely with numerous different departments to assess training and program needs and then find appropriate program options to fill those needs. The Designated Coordinator is responsible for the delivery and evaluation of services provided by the license holder including the provision of supervision, support, and evaluation of activities. You will also attend various meetings to gain a better understanding of the company goals, assist management with implementing new programs, and ensure all company programs are in line with our mission and goals. The ideal candidate has previous experience in a similar coordinator role, has excellent communication skills, and has the ability to multitask and switch priorities, as needed.

    Responsibilities

    Work closely with many departments, building relationships and learning the needs of each department;

    Oversight of the license holder's responsibilities assigned in the person's coordinated service and support plan and the coordinated service and support plan addendum;

    Assist and/or complete intakes with person-centered served plan with team members;

    Developing and/or writing and implementation of each client’s person-centered support plans, (CSSP) Coordinated Service and Support Plan, (SMA) Self-Management Assessment, and (IAPP) Individual Abuse Prevention Plan;

    Knowledge and understanding of behavior support and implementing positive support plans;

    Help develop, review, and monitor implementation of service outcome plans;

    Maintain all signatures as required by 245-D licensing rules and regulations;

    Maintain and update all files as required by 245-D licensing rules and regulations;

    Help develop, review, and monitor implementation of service outcome plans;

    Writing or quarterly and annual reports as needed or directed by the person served and/or team to fulfill person-centered plan;

    Oversee staff by providing continued support, training, and documentation regarding performance and policies;

    Assist staff with daily responsibilities as needed;

    Maintain the schedule for the house and fill shifts as needed;

    Monitor target symptoms, schedules appointments, and communicates with all team members as needed; and

    Be available to receive calls during and outside of work hours from staff with questions or during emergencies.

    Qualifications

    Ability to pass a background study conducted by the Department of Human Services (DHS); Qualify as a Designated Coordinator under 245D licensing rules; Excellent communication and interpersonal skills;

    Strong organizational skills;

    Familiarity with basic computer programs, such as the Microsoft Office suite;

    The ability to work well independently and within a team; and

    At least one year of supervisory experience; and · Baccalaureate degree in a field related to human services, and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older (1); or · Associate degree in a field related to human services, and two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older (2); or · Diploma in a field related to human services from an accredited postsecondary institution and three years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older (3).
