    Part-Time Designated Coordinator – Carver & Scott County Area (In‑Person)

    One Day at a Time is seeking a compassionate, detail‑oriented Part-Time Designated Coordinator to support services in the Carver and Scott County area. This position is in‑person at our main office, with some community-based responsibilities as needed. The ideal candidate values person-centered planning, strong communication, and building supportive relationships with individuals receiving 245D services.

    About the Role

    The Designated Coordinator (DC) ensures high-quality, compliant services by coordinating supports, maintaining documentation, and partnering closely with individuals, their teams, and our staff. This role offers meaningful work, consistency, and the opportunity to make a real impact.

    Key Responsibilities

    • Develop, update, and monitor CSSPs, Addendums, and progress documentation

    • Maintain compliance with MN DHS 245D standards

    • Communicate regularly with individuals, guardians, case managers, and DSPs

    • Support staff with training, expectations, and follow‑through

    • Conduct visits, observations, and person-centered planning meetings

    • Ensure timely documentation and follow-up on service needs

    • Promote a positive, supportive, and inclusive culture

    Required Qualifications (Per MN Statute 245D) To qualify for the Designated Coordinator role, an applicant must meet one of the following education and experience pathways:

    • Bachelor’s degree in a field related to human services, education, or health and 1 year of full‑time direct care experience with persons with disabilities or persons age 65+ (or equivalent experience providing care or education to vulnerable adults or children)

    • Associate degree in a field related to human services, education, or health and 2 years of full‑time direct care experience with persons with disabilities or persons age 65+ (or equivalent experience providing care or education to vulnerable adults or children)

    • Diploma from an accredited postsecondary institution in a field related to human services, education, or health and 3 years of full‑time direct care experience with persons with disabilities or persons age 65+ (or equivalent experience providing care or education to vulnerable adults or children) • Or 50 hours of education and training related to human services and disabilities AND 4 years of full‑time direct care experience with persons with disabilities or persons age 65+, completed under the supervision of someone who meets one of the qualification pathways listed above

    Applicants must also demonstrate knowledge of:

    • Person-centered planning

    • Positive support strategies

    • 245D service planning and documentation requirements

    • Coordinating and evaluating individual service delivery as required under 245D.081

    Additional Requirements:

    • Strong communication, organization, and follow‑through

    • Ability to work independently and collaboratively

    • Valid driver’s license and reliable transportation

    • Ability to maintain professional boundaries and uphold individual rights

    Schedule & Compensation

    • Part-time, in‑person

    • Tuesday, Wednesday, and Thursday — 8:00 AM to 4:00 PM

    • Competitive pay based on experience
