    We are actively hiring for new client cases across both Philadelphia and Montgomery County. Successful candidates must be willing to travel to client homes within these two service areas.

    Key Responsibilities

    Execute personalized care plans consistently, ensuring client safety and well-being.

    Provide all Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs).

    Coordinate with the nursing and management team to report any changes in client condition.

    Maintain accurate, digital documentation of all services provided per shift.

    Serve as a primary caregiver for assigned clients, fostering a trusted, long-term relationship.

    Requirements

    A genuine passion for caring for others and improving their quality of life.

    A high school diploma or G.E.D. (required).

    A CNA certification (preferred but not mandatory).

    Reliable transportation to visit clients when needed
