Provide Person-Centered Support: Engage individuals in meaningful activities based on their preferences, strengths, and goals outlined in their Individual Service Plans (ISPs).
Facilitate Community Integration: Accompany and support individuals during community outings, volunteer opportunities, and social engagements to promote independence and social inclusion.
Support Daily Activities: Assist with daily routines such as meal preparation, feeding, toileting, hygiene, and mobility while encouraging maximum independence.
Skill Development: Support individuals in developing and enhancing skills related to communication, self-care, vocational training, and social interactions.
Monitor and Document Progress: In accordance with regulatory and agency requirements, maintain accurate daily documentation of participation, behaviors, incidents, and progress toward goals in the Therap platform.
Ensure Safety and Well-Being: Implement safety protocols and provide supervision to ensure a safe, supportive, and respectful environment.
Implement Behavior Support Plans: Follow strategies and interventions as outlined in each individual’s behavior support plan to address and manage behavioral needs.
Collaborate with Team Members: Communicate effectively with coworkers, supervisors, clinicians, and families to coordinate support and report concerns or changes in condition.
Promote Dignity and Advocacy: Support each individual's rights, dignity, and choices and advocate for their needs and preferences.
