D Square Healthcare Services Inc is looking for a compassionate and dedicated Direct Support Professional to join our team in Shakopee, MN. Responsibilities:

Provide direct care and daily living support to individuals with intellectual or developmental disabilities

Assist clients with personal hygiene, grooming, meal preparation, and household tasks

Implement individualized support plans (ISPs) and document progress accurately

Transport and accompany clients to medical appointments, community activities, and errands

Administer medications and monitor health conditions per care plan guidelines

Foster independence, dignity, and community inclusion for each individual served

Communicate effectively with supervisors, families, and healthcare team members

Requirements:

High school diploma or GED equivalent required

Previous experience as a DSP, caregiver, CNA, or in a related healthcare/human services role preferred

Valid driver's license and reliable transportation

Ability to pass a background check as required by Minnesota DHS

CPR/First Aid certification (or willingness to obtain upon hire)

Strong communication, patience, and problem-solving skills

Genuine compassion and commitment to person-centered care
