Director of Case Management (RN)

We’re hiring a Director of Case Management in Sun City Center, Florida, who wants to lead a hospital-based team, influence patient outcomes, and advance their career in healthcare leadership.

You'll oversee case management operations at a 138-bed community hospital, guiding discharge planning, care coordination, regulatory compliance, and departmental performance. You'll have meaningful leadership responsibility while collaborating with hospital executives and interdisciplinary teams.

What makes you a great fit
• Bachelor's degree required; master's preferred
• Active RN or LCSW license; RN preferred
• 3+ years of acute hospital case management experience
• 2+ years of acute care case management leadership experience required
• Recent acute hospital experience within the past year
• Strong understanding of discharge planning, regulatory compliance, and interdisciplinary collaboration
• HCA experience is a significant plus
• Demonstrated leadership stability and commitment to team development

Compensation & Benefits
• Base salary: $102,107–$153,088 annually, based on experience
• Target annual bonus: 17.5%, with first payment in 2027
• Potential sign-on bonus for eligible non-HCA candidates
• Relocation assistance available
