Position Overview: The Director of Community & Healthcare Partnerships will be responsible for building and managing referral relationships with hospitals, rehabilitation centers, physician offices, and community organizations.

This role is ideal for someone who is confident in professional settings, understands how to build trust, and is motivated by results.

Responsibilities:

Develop and maintain strategic partnerships with local healthcare providers and community organizations.

Identify opportunities for collaboration to enhance service delivery and community health outcomes.

Lead outreach initiatives to promote our services and expand our client base.

Conduct needs assessments to inform program development and partnership strategies.

Facilitate communication between stakeholders to ensure alignment of goals and objectives.

Monitor and evaluate partnership performance and impact on community health.

Represent the organization at community events and healthcare forums.

Prepare reports and presentations to communicate partnership outcomes to leadership.

Requirements:

Bachelor's degree in Public Health, Healthcare Administration, or related field; Master's preferred.

2+ years of experience in healthcare partnerships or community outreach.

Strong understanding of the healthcare landscape and community health needs.

Excellent communication and interpersonal skills for stakeholder engagement.

Proven track record of successful project management and collaboration.

Ability to work independently and as part of a team in a fast-paced environment.

Proficient in Microsoft Office Suite and data analysis tools.
