    About the Opportunity

    Every year, thousands of Medicare patients experience preventable health declines simply because no one caught the warning signs in time. At ProMedix Health, we're changing that — combining clinical expertise with proprietary remote monitoring technology to catch problems before they become emergencies. We're a growth-stage virtual care platform partnering with primary care physician groups and healthcare systems nationwide, and we're looking for a Director of Nursing who wants to build the clinical program that makes this possible.

    This isn't a role where you inherit a mature department and keep the lights on. You'll be writing the playbook, the SOPs, the audit process, the training program from a blank page and then leading the team that runs on it. If you've ever looked at a broken workflow and thought "I could build this better," this is that job.

    What Success Looks Like in Year One

    You'll have built a fully staffed, audit-ready clinical team operating on documentation standards, escalation protocols, and quality processes that didn't exist when you started, with zero compliance lapses and a clear operating rhythm between your team and ordering physicians.

    What You'll Own

    Team Leadership Recruit, hire, and develop the RNs, LVNs, MAs, and Care Coordinators delivering virtual chronic care management to our enrolled Medicare patients. Design productivity and engagement standards from scratch, then own team performance as we scale.

    Clinical Quality & Compliance Build our documentation standards, internal audit process, and training program from the ground up, establishing what medical necessity documentation looks like here and ensuring it supports compliant billing. Partner with revenue cycle and billing teams so every submitted claim is backed by solid documentation.

    Physician Partnership Serve as the primary liaison between clinical staff and ordering physicians. Build the communication rhythm for care plan changes and escalations, and design the protocols for when and how your team reaches physicians.

    Patient Monitoring Infrastructure Build response protocols for RPM device alerts, including response time standards, and design the workflows that keep patient engagement on track.

    SOPs, Credentialing & Reporting Author the full set of clinical SOPs (onboarding, monitoring, care planning, escalation, disenrollment), build a credentialing tracking system with zero lapses, and deliver weekly performance reporting to leadership.

    As we grow toward a national footprint, this position is positioned to grow with it, the clinical infrastructure you build now becomes the foundation for a larger clinical operations function as we scale.

    What You Bring

    Required:

    Active RN license in California, in good standing

    BSN required; MSN preferred

    5+ years of nursing leadership experience, including direct supervision of RNs and clinical staff

    Clinical background in SNF, acute care, outpatient chronic disease management, or a comparable high acuity setting

    Strong working knowledge of CMS documentation standards and clinical compliance requirements

    A track record of building systems and processes from scratch, not just operating within existing ones

    Comfort with technology platforms and genuine interest in virtual care delivery

    Strong Plus (not required):

    Experience in RPM, CCM, CoCM, TCM, or other chronic care management programs

    Prior experience building or scaling a clinical program or department

    Telehealth or virtual care delivery background

    Familiarity with EHR systems in a care management or value-based care context

    A Quick Check

    This role will be a stretch if your background is primarily home health or assisted living, since the documentation culture and compliance environment here differ meaningfully from that setting. It's also built for someone who wants to create a program, not step into an established one, it's a fully on-site role in Santa Ana given how hands-on the first year of building this will be, so remote or hybrid arrangements aren't available.

    Compensation & Benefits

    $140,000 – $180,000 per year, based on experience and qualifications
