About the Role
We are an established, independently owned Medicare-certified home health agency in San Antonio that has been open since 1996 seeking a hands-on Director of Nursing to lead clinical operations, ensure compliance, and support continued growth.
This is not a corporate DON role. Our DON is actively involved in day-to-day clinical oversight, quality assurance, staff support, and survey readiness. We are looking for someone who values structure, accountability, and patient safety and understands the realities of home health.
Key Responsibilities
Provide overall clinical leadership and oversight for nursing services
Own and manage the QAPI program and quality improvement initiatives
Ensure compliance with Medicare Conditions of Participation, Texas HHSC rules, and agency policies
Oversee clinical documentation and support OASIS accuracy (coding outsourced; DON-level understanding required)
Supervise, mentor, and support RNs and LVNs
Participate in survey readiness and respond to audits as needed
Coordinate with intake, scheduling, and administrative staff
Be available for occasional field visits (spot checks, coverage when needed)
Support growth while maintaining compliance and clinical quality
Required Qualifications (Non-Negotiable)
Active Texas RN license in good standing
Medicare Home Health experience (minimum 2–3 years)
Prior leadership experience (DON, ADON, Clinical Manager, QA Nurse, or Supervisor)
Strong understanding of:
Medicare home health regulations
Clinical documentation standards
QA/QAPI processes
Comfortable working in a small-agency, hands-on environment
Strong organizational and communication skills
Preferred (Strongly Desired)
OASIS SOC/ROC/Discharge experience
Survey or audit experience
Psych or complex patient experience
Axxess, Kinnser, or similar home health EMR experience
Bilingual (English/Spanish)

Compensation
Salary range: $85,000 – $100,000 (based on experience)
Paid Time Off (PTO) offered
