Job Summary
We are seeking a compassionate, organized, and motivated Registered Nurse Supervisor to join our growing team. This role is ideal for an RN interested in leadership, community-based care, and contributing to the early growth of a rapidly expanding organization.
In this hybrid remote position, you will oversee client assessments, care planning, and clinical supervision while training and evaluating Home Health Aides (HHAs). You will work collaboratively with leadership to ensure high-quality, compliant, and patient-centered care. Flexible scheduling is a key benefit of this role.
Supervisory Responsibilities
Conduct in-home client assessments and develop individualized service plans
Update diagnoses and care plans within the Client Service Agreement and WellSky system
Lead onboarding and ongoing training for Home Health Aides (HHAs)
Perform HHA competency evaluations prior to client assignments, especially for clients with specialized or complex care needs
Provide clinical support and guidance to HHAs and client families
Address client and family concerns professionally and compassionately
Educate clients, caregivers, and families on aftercare and ongoing care practices
Promote and uphold high standards of care across the organization
Key Responsibilities
Organize, conduct, and document HHA training and continuing education
Ensure HHA compliance with training, competency, and regulatory requirements
Supervise personal care services for medically fragile or compromised clients
Complete, review, and update client service plans in coordination with care staff
Conduct supervisory home visits to assess client condition, satisfaction, and quality of care
Ensure services provided align with the authorized level of care
Maintain accurate and timely documentation of visits, assessments, training, and care plans
Provide guidance on complex or sensitive clinical cases
Consult with clients and authorized representatives regarding medical or care-related concerns
Participate in agency growth initiatives and community outreach during the early launch phase
Perform other duties as assigned by leadership

Qualifications
Bachelor’s Degree in Nursing (Required)
Active Registered Nurse (RN) license in the State of New Jersey (Required)
Minimum age of 21 years
Knowledge of local, state, and federal home care regulations
Ability to communicate effectively with clients, families, and interdisciplinary care teams (translation services available when needed)
Physical Requirements
Ability to sit for extended periods and work on a computer
Ability to lift up to 15 pounds occasionally
Compensation (All-Inclusive)
Initial Client Assessment: $90
Reassessment: $70
Operations & Management: $35/hour
Mileage Reimbursement:
$0.70 per mile for travel exceeding 40 total miles round-trip from home
