    ABOUT THE ORGANIZATION Open Arms & Heart Services Inc. is a well-established, high-volume Unskilled Home Health Agency based in Polk County, Florida, providing Personal Support, Companion, and Respite services across multiple counties. We are actively expanding into Skilled, Medicare-Certified Home Health Services and are seeking an experienced Registered Nurse (RN) Director of Nursing to lead this transition from unskilled to skilled operations.

    IMPORTANT ROLE EXPECTATION – TRANSITION PHASE This position will begin by supporting and overseeing clinical operations within our existing unskilled home health structure while simultaneously building and leading the transition into a Medicare-certified Skilled Home Health agency.

    The selected RN must be comfortable:

    • Working within an active unskilled agency during the startup phase
    • Building skilled systems while the unskilled operation continues
    • Leading the agency through accreditation and Medicare certification

    POSITION SUMMARY The Director of Nursing (DON) will serve as the Clinical Authority and Skilled Startup Leader, responsible for building, launching, and overseeing all Skilled Home Health operations, while supporting current unskilled clinical oversight during the transition.

    REQUIRED EXPERIENCE (NON-NEGOTIABLE)

    • Active Florida Registered Nurse (RN) License
    • Minimum 5 years of Skilled Home Health experience
    • Direct experience with at least ONE of the following:
    • Skilled agency startup
    • Medicare certification process
    • Federal Medicare survey preparation & pass
    • Hands-on experience with:
    • ACHC, CHAP, or Joint Commission Accreditation
    • Medicare Conditions of Participation
    • OASIS Documentation
    • 60-Day Physician Plans of Care
    • Skilled QA/QAPI Programs
    • Experience supervising RNs, LPNs, and Therapy disciplines (PT, OT, ST)
    • Thorough understanding of Skilled Infection Control, Emergency Preparedness, and Clinical Documentation Audits
    • Ability to pass Level 2 background screening and Drug screening

    KEY RESPONSIBILITIES

    • Serve as Clinical Authority for all Skilled Services
    • Lead the agency through ACHC Accreditation, CMS-855A Medicare Enrollment, and Federal Medicare Survey
    • Build and oversee OASIS workflows, Skilled clinical documentation systems, Infection Control Program, and Skilled QAPI Program
    • Supervise Field RNs, LPNs, and Skilled therapy contractors (PT, OT, ST)
    • Participate in Skilled admissions and discharges, Clinical audits and chart reviews, Survey preparation and Plans of Correction
    • Establish 24/7 RN on-call system and Skilled training and competency systems

    PREFERRED QUALIFICATIONS

    • Prior experience as Director of Nursing (DON), Clinical Manager, or Skilled Home Health Administrator (RN)
    • Experience with Medicare Advantage Plans and Dual-Eligible patients
    • EMR & OASIS systems: Axxess, Homecare Homebase, WellSky/Kinnser
