We are seeking a high-impact, strategic Director of Nursing to join a skilled nursing provider with two community-based SNFs in Tracy, CA.
These are permanent, full-time opportunities with a mission-driven operator focused on rebuilding and elevating clinical leadership. Both buildings are well-established with strong census, internal support, and proven reputations.
This is the ideal opportunity for a seasoned DON or a leadership-ready ADON looking to step into a high-autonomy role with the ability to make immediate operational impact.
You can expect a base salary of $145,000–$165,000, monthly performance bonuses (up to $15,000/year), and full health benefits. The position is hiring immediately for the right clinical and cultural fit.

Qualifications
Active RN license in California (required)
3+ years of experience as a Director of Nursing, or 4+ years as a strong ADON/unit manager
Proven leadership in team development, survey readiness, and skilled operations
Strong communication, coaching, and hiring/interviewing skills
Experience in high-acuity or high-skilled mix buildings preferred
Local to the Tracy, CA region or within a reasonable commuting distance (Stockton, Manteca, Modesto, etc.)
Responsibilities
Oversee day-to-day clinical operations at assigned SNF (Tracy, CA)
Hire, develop, and mentor nursing leadership including supervisors and licensed staff
Analyze quality metrics and implement plans to improve outcomes and survey results
Drive high standards of care in collaboration with internal stakeholders (DSD, case management, therapy, etc.)
Maintain regulatory compliance and proactively manage survey readiness
Build and sustain a culture of accountability, engagement, and patient-centered care
