    MVP Recruitment is proud to partner with a highly regarded Skilled Nursing and Rehab Center in the Westwood MA area to recruit a compassionate and experienced Director of Social Services.

    This full-time leadership role will support the Social Work department, with a primary focus on the Short-Term Rehabilitation Unit. The ideal candidate will bring strong clinical knowledge, excellent communication skills, and a passion for supporting patients and families through transitions of care.

    Position Overview:

    The Director of Social Services is responsible for leading the daily operations of the Social Services department, ensuring high-quality psychosocial care and effective discharge planning for residents—particularly those in short-term rehab. This role works closely with the interdisciplinary team and plays a key part in care coordination, family communication, and regulatory compliance.

    Key Responsibilities:

    • Supervise and support social work staff across the center

    • Conduct and oversee psychosocial assessments, care planning, and discharge coordination

    • Serve as the primary liaison between residents, families, and the clinical team

    • Coordinate post-discharge services such as home care, outpatient therapy, or long-term placement

    • Address psychosocial concerns, grievances, and provide counseling as appropriate

    • Ensure accurate and timely documentation in accordance with state and federal guidelines

    • Participate in interdisciplinary meetings and quality improvement initiatives

    • Provide education, mentorship, and performance oversight for the social work team

    Qualifications:

    • Bachelor’s or Master’s degree in Social Work (BSW or MSW)

    • Current MA LCSW or LICSW preferred

    • Minimum of 3 years of experience in a Skilled Nursing or Post-Acute Rehab setting

    • Prior supervisory or management experience preferred

    • Strong understanding of discharge planning and community resources

    • Excellent communication, documentation, and organizational skills
