Position Title: School Social Worker- Maternity Leave
F.T.E: 1.0, Temporary, April-June 2021
Location: C3820 N Spaulding Ave, Chicago, IL 60618
Reports To: School Director

Position Description:
A Distinctive Schools School Social Worker collaborates with school teams to provide individual and small group counseling services and life skills development in support of students' social and emotional well-being. This person will:
Work with school administration and teachers to develop, support, and monitor the implementation of formal and informal behavior plans
Observe general education and special education students in the classroom to assist the teacher with behavioral or emotional interventions and recommend strategies to support student social development
Maintain professionally written records for all students including assessment protocols, treatment plans, session notes, progress reports and correspondence
Attend all IEP meetings and communicate treatment plans to key stakeholders
Requirements:
Master’s Degree in Social Work
School Social Work License/Endorsement
