    Dockside Services, Inc. Team is seeking a dynamic individual to join our team as the Case Manager maintains direct/indirect contact with clients and parents, while implementing identified interventions and training.

    Job Requirements

    Collected evidence, interviewed clients and prepared necessary reports for case management

    Assessed and reassessed the risk and safety concerns relating to the children in their homes and/or foster homes

    Developed case plans that established appropriate permanency goals for children in a timely manner

    Coordinated and monitored children in out-of-home placements

    Facilitated face-to-face contact for parent-child visitation

    Identified and addressed clients’ physical and mental health/behavioral needs

    Collaborated with community partners to ensure enhanced customer access to supportive services

    Qualifications

    Rapt and Safe Home certification preferred

    2-5 years of experience preferred
