    Dockside Services, Inc. Team is seeking a dynamic individual to join our team as the Case Manager –Part-Time maintains direct physical contact with clients while implementing the interventions designated on a client’s master treatment plan.

    Duties and Responsibilities

    Implements case management plan according to master treatment plan.

    Provides on-call services to clients up to 24 hours per day, 7 days per week.

    Provides accurate and timely documentation of contacts for client’s treatment; including progress notes of daily contact, weekly (client) schedules, school correspondence, group facilitation, and documentation of all case management interventions related to a client’s master treatment plan.

    Attends weekly supervision and/or staffing meetings for information-sharing with therapists, supervisors, and other team members; comes prepared to participate.

    Demonstrates accurate knowledge of interventions that are program-specific in relation to each program of Dockside Services, including home based, foster care, substance abuse, and visitation.

    Maintains a balanced and professional relationship through dress, personal contact, written documentation, and communication that exemplifies the conduct expected by Dockside Services.

    May participate in referral related meetings/needs for the caseload which can include court, school/education, CFTM meetings, etc.

    May participate in compliance-related activities, such as chart reviews and reviewing documentation for file compliance

    May involve the provision of case management and/or training, and home studies leading to the licensure of Foster Parents and child placements under the supervision of the Regional Director and/or Foster Care Coordinator. The case manager will not make decisions concerning child placements or foster parent licensing.

    Provides safe and timely transport of clients as needed for groups, appointments, visitation, etc.

    Represents Dockside Services’ work ethic, including communication with client families, outside agencies, referral sources, and team members, in a professional manner

    Maintains certification for employment requirements as directed by Dockside Services.

    Trains and mentors new employees as requested by Regional Director or Supervisor.

    Becomes proficient in all required skills during probationary period as mandated by agency policy.

    Qualifications

    Education and/or Experience

    Bachelor’s degree from an accredited college or university and 3 years of experience working in a child placing agency for Foster Care Services; and 3 years of experience working with child welfare for Community Based Services.

    Certificates, Licenses, and Other Qualifications

    Valid state driver’s license in good standing

    Appropriate automobile insurance (at least state minimum)

    Certificates and licensure must be consistent with State and Federal requirements.
