POSITION SUMMARY
The APC Nurse Practitioner (APCNP) functions in an Advanced Practice Role as a clinical expert and independently performs and documents a complete physical examination for acute and complex chronically ill patients. The APCNP administers medical care to assigned population. The APCNP administers preventive care and chronic illness surveillance. The APCNP’s responsibilities include assessment, diagnoses and management of health-illness needs. The primary goal is to deliver Age Friendly Health Systems by focusing on geriatric principles, including the 4M’s (What Matters, Medication, Mentation and Mobility) using evidence based practices in the care for older adults.

- Demonstrates expertise in the 4M’s: What Matters, Medication, Mentation, Mobility in caring for older adults
- Advocates for “What Matters” for the patient, documenting, communicating and applying patient’s wishes to guide their care
- Provides socioeconomic resources and coordinate interdisciplinary care, including case manager and social worker team members.

- Recognizes and proactively improve gaps in care and communication
- Demonstrated expertise in assessment, intervention, and patient care management.
- Provides health education, prevention, diagnosis and treatment of illnesses for acute, critical and complex ill patients

Required Experience
EDUCATION/EXPERIENCE/TRAINING
- Current California Nurse Practitioner License
- Master’s degree in nursing from an accredited nurse practitioner program
- Board Certification: Board certified as a Nurse Practitioner
- Two to five (2-5) years previous experience in chronic care/palliative care outpatient setting
- Organizational skills and personal leadership qualities including tact and diplomacy, necessary to coordinate all aspects of the program and to integrate the efforts of the hospital, medical staff, and program staff
- Evidence of continuing education and self-development to support license requirements
- Current Basic Life Support Card
- California Driver’s License and current car insurance required
