    The long-term care nurse works in a long-term care facility with patients with chronic physical and/or mental disorders, who are primarily elderly. Responsible for the day-to-day care of patients, assisting in operation of the facility, staff supervision, assessing program quality, program growth and development, and service excellence often are responsibilities of the long-term care nurse. Working in long-term care requires working with patients with challenging diagnoses. Practice settings include nursing homes, skilled nursing facilities, Community Living Center (CLC)

    Community Living Center (CLC): RNs assigned to work on community living center will be responsible for patient assessments, medication administration, IV therapy, interdisciplinary team coordination, admissions, discharges, internal and external transfers, delegating to assistive personnel, electronic medical record documentation, patient transport, and emergency response procedures.

    Years of Experience in the Specialty:

    • Minimum three years current (must have worked within the last six months) experience in a long-term care setting.

    Minimum Education Requirement:

    • Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation
    • Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). Bachelor’s Degree in Nursing or in related field or a Master’s degree in Nursing or in related field is highly preferred.

    The minimum requirement for all registered nurses shall include:

    • A current unrestricted registered nurse license from the state of Virginia (or reciprocal state) (United States).
    • BLS (American Heart Association)
    • Ability to provide three (3) Supervisory references directly related to experience in this role.
    • Ability to pass federal healthcare credentialing standards

    Long Term Care Nurse responsibilities may include but not limited to:

    • Assuming a patient care assignment for a group of patients
    • Assessment and care of patients with a wide range of physical and psychological
    • Diagnoses and with a wide range of treatment modalities
    • Care of patients requiring special precautions such as isolation
    • Inserting and maintenance of peripheral lines
    • Maintaining central lines for administration of intravenous fluid
    • Providing emergency care as indicated
    • Administration of medications (oral, subcutaneous, intramuscular and intravenously) and blood products
    • Verification of physician orders
    • Complete and accurate documentation of care according to facility/unit policies
    • Phlebotomy and bedside testing
    • Psychological support of patients as indicated
    • Complex wound dressing care and maintenance
