RN/LPN Responsibilities
• Provide skilled in-home nursing care for low- to high-acuity patients• Complete patient assessments and follow individualized care plans• Administer medications and treatments as prescribed• Collaborate with families and interdisciplinary team members• Accurately document care and report changes to the RN Clinical Supervisor/Site Director• Safely assist with lifting, positioning, and transferring patients
Qualifications
• Active Arizona RN or LPN license (or Compact License)• Valid Arizona Driver’s License• Current BLS Certification (online-only courses not accepted)• Current TB test or chest X-ray within the past 12 months• Level One Fingerprint Clearance Card• Article 9 Training (assistance available)• Strong communication, time management, and problem-solving skills
