    Position: CSBG Case Manager Job Status: Grade 10, Step depending on experience and qualifications Classification: Non-Exempt; 12-month employee Supervisor: Community Services Program Director The individual selected for the position must be able to perform the essential functions of the position either with or without a reasonable accommodation.

    General Responsibilities:

    It is the responsibility of all EOAC employees to support EOAC’s mission and goals, to respect the confidentiality of all who come to us for information or assistance, and to personally maintain the dignity and integrity of one who is placed in a position of public trust.     The Community Services Program offers a wide range of services, including the Community Services Block Grant (CSBG), to income-eligible community members; help them transition out of poverty and into self-sufficiency.

    Essential Functions:

    1. Maintain confidentiality of client/agency information as required by State and Federal laws, appropriate regulations, and professional practice

    2. Ability to communicate effectively in written and/or verbal form to individuals and/or groups with a clear and concise explanation of EOAC processes and services.

    3. Must consistently demonstrate and communicate EOAC’s Mission and Vision.

    4. Must consistently demonstrate excellent customer service with a compassionate attitude towards all clients providing multi-services offered by EOAC to qualifying clients and their families.

    5. Processes applications and/or interviews to determine eligibility for assistance according to regulations set forth by the funding source and EOAC. Ensures funds are available prior to initiating a payment voucher. Ensures pledges made do not exceed budgeted funds for that county.

    6. Collects and maintains accurate and complete data for client files as required by the funding source and EOAC; adheres to all Federal and State requirements. Promptly enters collected data into tracking system.

    7. Conduct professional case management of clients identified as eligible; assist clients in transferring out of poverty by identifying and removing barriers; develop action plans, mentor clients to achieve goals set forth in action plans, and follow-up to determine reportable self-sufficiency outcomes; act as contact person for counseling services on an as-needed basis; provide referrals as needed.

    8. Attend community meetings/events as assigned, staff meetings, and trainings.

    9. Conduct outreach to targeted population.

    10. Ensure all services and outcomes have been documented and submit a written report to Director by the 2 business day of every month.

    11. Provides timely feedback to the agency regarding service failures or customer concerns.

    12. Process assistance for other grant-funded services as needed and as customers are eligible. Creates utility payment vouchers for processing, ensuring all required back-up documentation is available and follows accounting office directives; utilizes program data entry procedures as required by the funding source and EOAC to make accurate and timely documentation of all services delivered; contacts utility vendors on behalf of clients to make pledges/create holds to ensure continued utility service or to restore utility service.

    Competencies:

    1. Must have good organizational skills and proven ability to work within timelines to meet demands. Must be able to prioritize multiple tasks.

    2. Intermediate skill and familiarity with Microsoft Office Applications: Word, Excel and Outlook.

    3. Participate in team activities, mandatory meetings and trainings, and meet the service delivery standards set forth by the Community Services Program and EOAC.

    4. Provide Information and Referrals to other EOAC services and/or other community services as need by applicants.

    5. Ability to work independently with flexibility within a team environment; ability and willingness to work as a team member to support EOAC’s mission and goals.

    6. Maintain excellent professional relationships with vendors, and community partners.

    Work Environment

    1. This job operates in a professional office environment with ambient lighting, and climate control.

    2. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.

    3. Prolonged sitting.

    4. This job works with clients who are in high stress situations; ability to de-escalate tense situations is necessary.

    Physical Demands The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.

    1. While performing the duties of this job, the employee is regularly required to talk or hear customers in-person, and over-the-phone.

    2. The employee frequently is required to stand; walk; use hands to handle or feel; and reach with hands and arms.

    3. This is a largely sedentary role; however, some filing is required. This would require the ability to lift files, open filing cabinets and bend or stand on a stool as necessary. Individuals may need to sit or stand as needed. This position may require walking primarily on a level surface for periods of time as well as reaching above shoulder heights or below the waist or lifting as required to file documents or store materials throughout the workday. Proper lifting techniques are required. This position may occasionally lift up to 25 pounds for files and computer printouts.

    Minimum Qualifications:

    1. High School Diploma, GED or Equivalent and at least two (2) years of college level coursework in social services or related field.

    2. OR

    3. High School Diploma, GED or Equivalent and at least three (3) years of experience in social work or human services field.

    4. Must be able to pass Criminal History check.

    5. A valid Texas driver’s license or ID card is required.

    6. Must be able to pass pre-placement physical assessment.

    Travel:

    In-State travel as required for training and client visits. Minimal Nationwide travel.

    Preferred Qualifications:

    Bilingual (Spanish)

    Other Duties:

    Perform all other duties as deemed necessary.

    Work Schedule:

    This is a full-time position. Occasional evening and weekend work may be required as job duties demand.
