    BASIC FUNCTION:

    Erie Neighborhood House, a community-building agency empowering Chicago’s low-income and immigrant families, and dedicated to promoting a just and inclusive society is seeking an EXPERIENCED PROFESSIONAL TO SERVE AS THE COMMUNITY RESOURCE COORDINATOR.  Proyecto Cuídate provides individual, group and family services to support, inform and educate the community on topics that strengthen the individual and family unit. Proyecto Cuídate programming includes:

    • •Informative workshops and trainings in areas such as violence prevention, health & wellness, parenting skills, etc.,
    • •Supportive counseling, screenings, referrals and case management,
    • •School based and after school youth programming

    POSITION RESPONSIBILITIES:

    • Coordinate, develop, facilitate and evaluate parenting classes and women empowerment groups,

    • Provide program workshops, training, technical assistance and support to In House and partner agencies.

    • Plans and coordinates community outreach and other recruitment activities through the development and maintenance of partnerships and collaborations in the community.

    • Conduct psycho-social intake assessments, provide crisis intervention support to clients and referrals to other supportive agencies as appropriate.

    • Supports staff with home visits and attends as necessary.

    • Provides supervision to assigned staff, providing administrative, education and reflective supervision.

    • Support clinical staff with maintaining their files and provide case management support including care coordination, making appointments, accompaniment and follow ups for psychological evaluations, assessments and medication management.

    • Creates and maintain client files in accordance with program documentation requirements.

    • Supports program staff in inputting data entry in a timely manner.

    • Represent Erie Neighborhood House to the public and actively participant in community meetings and events as required.

    • Attends monthly department, all staff, funder and other required meetings.

    • Performs other duties as required.

    POSITION REQUIREMENTS:

    • A Bachelor’s Degree in Social Work, Psychology, Child Development, or related field, or currently in school obtaining such degree. Master’s and LSW preferred.
    • A minimum of two years’ experience working in direct service to the community, particularly youth, adults and immigrant families.
    • Experience in program development and implementation.
    • Knowledge of care coordination and case management.
    • English/Spanish fluency.
    • Familiarity with domestic violence, child abuse, emotional and mental health issues.
    • Commitment to social justice and sensitivity to the assets and challenges of a diverse group of participants. 
    • Demonstrated organizational skills.
    • Be self-motivated, self-directed, creative, reliable and dependable.
    • Exhibit sincere interest in working with youth and families.
    • Completion of fingerprint background check.
    • Ability to work a flexible schedule, including evenings, and travel to off-site locations.
    • Superior communication, word processing, and management skills.

    Compensation Range: $36,000-$40,000, plus excellent benefit package
