BASIC FUNCTION:
Erie Neighborhood House, a community-building agency empowering Chicago’s low-income and immigrant families, and dedicated to promoting a just and inclusive society is seeking an EXPERIENCED PROFESSIONAL TO SERVE AS THE COMMUNITY RESOURCE COORDINATOR. Proyecto Cuídate provides individual, group and family services to support, inform and educate the community on topics that strengthen the individual and family unit. Proyecto Cuídate programming includes:
POSITION RESPONSIBILITIES:
Coordinate, develop, facilitate and evaluate parenting classes and women empowerment groups,
Provide program workshops, training, technical assistance and support to In House and partner agencies.
Plans and coordinates community outreach and other recruitment activities through the development and maintenance of partnerships and collaborations in the community.
Conduct psycho-social intake assessments, provide crisis intervention support to clients and referrals to other supportive agencies as appropriate.
Supports staff with home visits and attends as necessary.
Provides supervision to assigned staff, providing administrative, education and reflective supervision.
Support clinical staff with maintaining their files and provide case management support including care coordination, making appointments, accompaniment and follow ups for psychological evaluations, assessments and medication management.
Creates and maintain client files in accordance with program documentation requirements.
Supports program staff in inputting data entry in a timely manner.
Represent Erie Neighborhood House to the public and actively participant in community meetings and events as required.
Attends monthly department, all staff, funder and other required meetings.
Performs other duties as required.
POSITION REQUIREMENTS:
Compensation Range: $36,000-$40,000, plus excellent benefit package
