Eastern Iowa Health Center is seeking a full-time Outreach Social Worker dedicated to supporting the homeless population through partnerships with local community organizations. This position is located at our Family Medicine Clinic, with a periodic responsibility at other outreach locations.

ESSENTIAL DUTIES AND RESPONSIBILITES

• Educate patients on community resources and complete appropriate referrals.
• Consult with clinical staff to evaluate and support the medical, psychological, and social needs of client.
• Establish community outreach relationships with neighborhood organizations to facilitate awareness and utilization of EIHC and its services.
• Fulfills outreach activities focused on the homeless population that meet HRSA requirements.

REQUIRED QUALIFICATIONS

• Bachelor’s degree in Social Work (BSW)
• Licensed in Iowa as LBSW- or ability to obtain license within designated timeframe.
