Part time Registered Dietician (20 HOURS PER WEEK)
Registered dietician to work in the outpatient area to perform nutrition screens, assessments, instructional care plans, counseling, and follow up with the focus on diabetes.
As a mandated part of the instructional team, responsible for patient education / nutrition support of patient with diabetes in group classes or one to one consultation.
Responsible for the instruction of Diabetes Self Management Education as outlined by the National Standards for DSME.
Performance expectations of a Certified Diabetes Educator with or upon receiving CDE credential.
Ability to perform oversight of the daily operations of the Diabetes Education Department when the program coordinator is not present.
Accurately schedules Diabetes Education outpatient appointments with appropriate clinician, either for one to one consultation or group classes.
Performs patient / family Diabetes Education Program assessment demonstrating effective interviewing skills and techniques.
Confidentially collaborates and accurately reports patient information.
Contributes legibly and accurately to assessment intake record, individual and group teaching record, and physician referral sheet.
Develops, instructs, and evaluates individual diabetes education plan, including fingerstick blood glucose testing, in accordance with current diabetes care and treatment standards, age specific guidelines as well as religious / cultural practices.
Develops, instructs, and evaluates individual Medical Nutrition Therapy plan in accordance with current diabetes care and treatment standards, age specific guidelines as well as religious / cultural practices.
Collaboration with diabetes education staff in measuring clinical and behavioral outcomes.

Required Skills
BS or MS degree in clinical nutrition / dietetics from an approved ADA university with completion of internship, CUP program or Title IV ADA program.
Registered Dietitian CDR required.
Must possess CT. RD State License.
Minimum practice experience - 1 year required in Diabetes Management Counseling
Certification in Diabetes Education (CDE) required or CDE eligible to be obtained within one year of hire.
Required Experience
BS or MS degree in clinical nutrition / dietetics from an approved ADA university with completion of internship, CUP program or Title IV ADA program.
Registered Dietitian CDR required.
Must possess CT. RD State License.
