    Full time Registered Dietitian 

    The Registered Dietitian will function as a member of the professional health care team providing clinical nutrition care at the SHMG Diabetes and Endocrinology practice and provide nutrition education to patients of the SHMG Diabetes and Endocrinology and the SHMG Medical Weight Loss Program.

     

     

     

    • Serves as one of the primary nutrition resources at the SHMG Diabetes and Endocrinology for the Medical Weight Loss Program.
    • Discusses all nutrition recommendations with SHMG Diabetes and Endocrinology and SHMG Medical Weight Loss Program physicians and serves under their medical supervision.
    • Educates and counsels patients, families and/or caretakers on recommended medical nutrition therapies (which includes the Center’s meal replacement program, if applicable) through individual counseling, group classes and/or community presentations.
    • Formulates individualized nutrition care plans based on assessment of patient needs and in consultation with the physicians.
    • Documents patient nutrition assessment, education and progress notes in the patient’s medical record
    • Serves as the primary point of contact for all nutrition consult referral requests from healthcare team and schedules nutrition visits on a timely basis utilizing tools to register patients.
    • Conduct nutrition education classes
    • Participates as an integral member of the interdisciplinary team by interacting with other members of the patient’s healthcare team.
    • Participates in SHMG Medical Weight Loss Program team rounds.
    • Represents Stamford Health by participating in relevant community events.
    • Participates in research studies, performance improvement projects and special projects as needed.
    • Stays current on the latest evidence-based nutrition care through participation in on-going professional development
    • Involved with on-going development of policies and procedures and nutrition standards for outpatient nutrition education.
    • Adhere to recognized standards of practice and standards of professional performance for registered dietitians as defined by the Academy of Nutrition and dietetics

    Required Skills

    EDUCATION:

    Bachelors Degree or Masters Degree in Nutrition/Dietetics or in Masters Program with Clinical requirements completed.

    Licensing/Certification/Registration: Registration, Type RD, Certification     Type CD-N

    Must have RD and be eligible for CD-N and successfully obtain within 6 months of employment

    YEARS OF EXPERIENCE:

    Minimum of 1-2 years of experience

    KNOWLEDGE:

    Requires knowledge of different disease states and medical nutrition therapy provided for those individualized patient needs.

    Familiarity with the Nutrition Care Process. Meal Replacement modalities and Electronic Health Records.

    Strong comprehension of the principles of clinical nutrition practice.

    Ongoing and continued education with the evolution of Meal Replacement Modalities
