Part-Time Social Worker
Position Summary: To coordinate, supervise, and provide for the social service needs of participants and their family members or caregivers. The social worker reports to the Center Director. Salary: $24.00 per hour Hours: 24 hours per week Essential Functions: - Assisting in the screening and interviewing of referrals to determine the general appropriateness of the participant for enrollment. - Evaluate the social service needs of participants and caregivers. - Assess and re-evaluate participants quarterly (ADCAPS) and as needed. - Coordination of care plan meetings. Include all inter-disciplinary team members. Document discussion during meeting and chart to files. - Assist in performing intakes and determine the appropriateness of referral for admission as needed. - Document changes in participant progress notes. - Serve as a member of the multi-disciplinary team in preparing and evaluating participant plans of care. - Provide input into daily and monthly activity sessions, care plan reviews, and the general program. - Serve as liaison between center staff, participants, families, and/or caregivers. - Maintain ongoing contact with referral sources within the community - Maintain regular contact with caregivers concerning the participant and the care they receive at home and at the center; visit homes of participants as necessary. - Recommend and assist participants and their caregivers in acquiring the services they need. - Provide guidance to the multi-disciplinary team regarding discharge planning and referral services including discharge procedures, referral, and follow-up, discharge summary with post Discharge goals, a recommendation for continuing care, and referral to appropriate community service agencies to support a participant's return to more independent living. Qualifications: Education/Experience: - Bachelor of Science degree in Social Work. - Current State of Maryland Social Worker License. - At least one year of experience in a professional capacity providing services to adults in a health care setting. - Demonstrated ability to interact positively with participants, caregivers, and other employees. - Understanding the physical, mental, psychological, and social needs of the elderly and adults with disabilities. - Ability to develop and maintain community relations. Language Ability: - Proficiency in reading, writing, and speaking the English language. - Ability to interact effectively and professionally with participants, families, and community referral sources. Reasoning Ability: - Must be able to follow instructions quickly and easily and seek out direction promptly. - Must be able to handle multiple tasks. - Must be able to work with minimal supervision after instruction is provided. - Must be aware of potential hazards or problems within the environment and seek out supervision promptly. - Must be aware of the confidentiality of client information. - Must take initiative and problem-solve.
