We're looking for a Nurse Practitioner to join our Clinical team as an independent remote contractor. This role will report to our Chief Medical Officer.

    EasyHealth Medical Solutions completes Annual Wellness Visits for Medicare members/patients of primary care physicians, medical groups, and health plans we serve. The Nurse Practitioner will perform comprehensive assessments which are compliant with the annual wellness visit, in a home setting, in a clinic setting, via telemedicine or in long term facility.     WHAT YOU'LL DO:

    • Complete a detailed evaluation of the medical and social assessment of the beneficiary members using EasyHealth documentation tool or client EMR.

    • Identify and refer members/patients for routine, urgent, and emergent case management.

    • Make recommendations for member's follow-up with their primary care physician.

    • Travel throughout local counties, regions, and/or states where you hold a current license.

    • Not required to prescribe or alter the members current treatment plans.

    • Stewardship - confirm beneficiary information and conduct home/clinic visit as per assignment. Maintain flexibility in schedule and please be available to assist with unexpected emergencies. Attend and participate in scheduled staff meetings, make judicious use of supplies and equipment.

    • HIPAA - maintain comprehensive understanding related to protected health information and observe HIPAA requirements.

    • Professionalism - maintain warm, caring, and courteous behavior with the members.

    • Performance - complete a comprehensive exam. The diagnoses listed in the documentation must be supported. Discuss care guidelines with the member. Escalate emergent and urgent condition appropriately as per the guidelines.

    • Member satisfaction - make sure that the member is satisfied with the exam and overall visit.

    • Member Communication- Maintain member privacy and courteous communication with members and colleagues.

    • Safety - maintain safe working environment. Follow safety policy and procedures listed subsequently.

    • Travel will be required as part of this role roughly 20% of the time

    WHO YOU ARE:

    • 2 years hands on experience as a Nurse Practitioner or Physician Assistant

    • Willing to work at least 16 hours a week

    • Current, unrestricted license in the state or the ability to acquire one

    • Must be board certified, preferably geriatric, or adult medicine specialty.

    • Familiarity with HCC model and quality HEDIS and other STAR measures

    • Effectively communicate with elderly and chronically ill members

    • Comfortable using Telemedicine tools and working in an office or home setting when required to do so.

    • Proficient in use of computer, tablet, and mobile technology
