    Job Title: Geriatric Care Manager/Senior Care Consultant/Nurse Care Manager

    Location: Fairfax County, VA

    Compensation: $70K - $90K annually depending on experience (full-time)

A background in social work, nursing, life enrichment coordinator in senior living environments, occupational therapy, counseling, and care coordination for older adults aligns beautifully with this role!

    Requirements:

    Qualified candidates need to hold a master’s degree in related field or a bachelor’s with 4+ years of experience in Gerontology (AL, SNF, CCRC, Home Care)

    Must have a car and valid driver's license to visit clients in Northern Virginia/DC metro area.

    Duties:

    Care Coordination

    Managing home health aides

    Medical oversight

    Interfacing with medical personnel

    Advocacy, information and referrals

    Complete comprehensive assessments

    Design individualized working care plans

    Coordinate, manage, and oversee care

    Experience in home care and/or skilled nursing/assisted living communities

    Accompany (drive) clients to physician appointments

    Advocate for client needs in-home or facility

    Communicate with those involved in client’s care

    Contribute knowledge base to ElderTree team

    Support other ElderTree team members

    Willing to take on-call rotation

    Visit clients in their homes, assisted living, and skilled nursing

    Perform quarterly client file audits, daily charting

    80% field work with clients, 20% remote work (charting/phone calls)

    Physical Requirements: ability to push a wheelchair, fold and place a wheelchair into car, and stand by assistance with transfers

    Ability to provide non-directive guidance and facilitate constructive relationships.

    Ability to ensure inappropriate placements, duplication of services, and unnecessary hospitalizations are avoided.

    Manage time efficiently.

    Ability to provide coordinated communication between family members, doctors and other professionals, and service providers.
