Responsibilities: - Provide continuous job support and fading in the community; - Cultivate and maintain appropriate networking sources with the community; - Maintain open communication with the individuals' employment sites; - Attend and participate in all relevant agency events, workshops, training and meetings; - Prepare written reports in the appropriate format for the referral source on a monthly, quarterly, or as-needed basis; - Assist clients with application process and interviews, and coordinate arrangements for job placement; - Monitor and evaluate client work performance, collect skill acquisition and production data, obtain feedback from client, employer, and coworkers, and provide additional training or intervention as needed; - Assess client skills and abilities and requirements of specific employment situations for the purpose of job/client matching; - Provide current information on clients via the vocational profile form; - Provide on-going assessment and follow-along services as needed and authorized; - Prepare and update written documentation for clients, employers, and counselors in a timely manner; - Provide job site consultations and backup support as needed; - Participate in the process of achieving and maintaining CARF accreditation; - Perform other duties as assigned. Requirements: - Communicate effectively with clients and staff verbally and in written form. - Meet the personnel requirements for Criminal History, CPS, DMV record checks, and HHS-OIG. - Obtain an evaluation indicating the absence of TB in a communicable form. - Availability to work various hours and shifts, both day and night. - Possess and maintain a valid driver's license. - Position may require lifting, running, and kneeling, and ability to lift up to 50 pounds.
