We are seeking a skilled Epic Ambulatory with Healthy Planet Analyst to support the implementation, optimization, and deployment of Population Health Management solutions.

Responsibilities:

• Develop and optimize Epic Healthy Planet applications to support clinical and operational goals.
• Deploy Population Health Management systems across Epic and non-Epic sites.
• Collaborate with stakeholders to implement clinically integrated network (CIN) solutions and quality measures.
• Certification in Epic Healthy Planet.
• Familiarity with Population Health Management, ACOs, and CIN workflows.
• Experience in implementing and supporting Epic Ambulatory application
