    We are looking for two Team Managers to add to our EverHeart Hospice team. We are looking for a Team Manager to oversee our homecare staff, and another for our Inpatient Care Center located on the 4th floor of Wayne Healthcare. • Inpatient Care Center Team Manager The typical hours for these positions are 8:00 am-4:30 pm , Monday through Friday, with potential for administrative on-call. Education and Experience: • Graduation from an approved School of Nursing. • Current RN licensure in the State of Ohio and the eligibility to obtain licensure in the State of Indiana or current RN licensure in the State of Indiana and the eligibility to obtain licensure in the State of Ohio. • Minimum of three years of experience in Medical/Surgical, Oncology or community health nursing. • Hospice experience preferred. • Minimum of one year of management experience. Responsibilities: • Assist with the development and implementation of policies and procedures • Manage departmental budget and control expenditures • Enforce departmental vision, goals, and implementation plan • Build and maintain an effective team • Participate in the Agency survey. (i.e., Indiana, Ohio, Joint Commission, Finance, etc.) • Provide expert leadership regarding optimum symptom management and overall care provided • Collaborate with the Medical Director, primary physicians, and/or consulting physicians regarding appropriateness for re-admission or re-certification and regarding appropriate symptom management • Participate in Interdisciplinary Team Conferences • Participate in the development, review, and evaluation of the patient’s Plan of Care in the GIP setting through ongoing review of clinical records and chart reviews • Monitor the quality of care and appropriateness of care provided in the inpatient setting • Monitor clinical documentation for timeliness, appropriateness, and compliance with Agency policy, accreditation bodies, and governing requirements • Assist the Director of Nursing with additional documentation requests from the Payer Service • Evaluate staff productivity. Make supervisory visits to evaluate staff competency annually • Participate in patient-nurse visits as needed • Work evenings and weekends as administrative on-call • Complete all documentation as required by the organization in compliance with Agency policy, accreditation bodies, and governing requirements • Other duties as assigned Qualifications: • Education: Bachelor’s degree in nursing or an associate degree with a minimum of five years of relative experience • Current RN licensure in the State of Ohio OR Indiana, and eligibility to obtain licensure in the other state • At least three years of experience in Medical/Surgical, Oncology, or Community Health Nursing • Preferred Hospice experience • Minimum of one year of management experience
