Duties and Responsibilities

Help families make healthcare choices that are best suited for the patientConduct psychosocial assessments to determine the best hospice care solutions for patientsLiaison with patients and other medical professionalsDocument the patient's wishes and needs accuratelyContact agencies and alternative programs and speak on behalf of patients and their familiesProvide emotional support, guidance and help resolve psychological crisesIntervene and resolve conflicts between families and the care providersProvide patients and their families with helpful resourcesAssist patients with their medical insurance documentationAssist families with planning for funeralsEducate families on hospice care processes and update them on loved ones' care and well-being

Job Types: Part-time

Salary: $85 per visit

Torrance, CA: Reliably commute or planning to relocate before starting work (Required)

Requirement:

MSW (Master's in Social Work)

Experience:

Social work: 1 year (Preferred)
